Your First Appointment
Prior to your first appointment, you will need to complete a Nutrition Assessment (2 options - fill out online or print and fill out), and an Informed Consent if you want us to talk to anyone about your treatment. If you want to pay by credit card, you will also need to complete the Electronic Payment Authorization form.
You can either arrive 20 minutes early, or you can download the forms below, complete them, and bring them with you.
For body image / disordered eating / eating disorder / weight concerns, complete these forms:
Disordered Eating Nutrition Assessment (print then fill out) or
Disordered Eating Nutrition Assessment (fill out online then print)
Informed Consent
Electronic Payment Authorization (for credit card payment)
For any other concerns (sports nutrition with no body image or weight issues, diabetes, renal, etc), complete:
Medical Nutrition Therapy Assessment
Informed Consent
Electronic Payment Authorization (for credit card payment)
If you have the results of any medical tests that are pertinent, please make sure to bring them. You do not need anything else for your first appointment.
